Courthouse in Coquille, Room 106. “If you bring your card, we can bill Medicare, Oregon Health Plan, Blue Cross-Blue Shield of Oregon, Pacific Source, Lifewise, Providence. For those paying by cash or check, the discounted price for a Trivalent Flu Shot (3 strains) is $25 and a Quadrivalent Flu Shot (4 Strains) is $35.
