On Tuesday, August 27, 2019 at approximately 4:10 A.M. Oregon State Police and emergency personnel responded to a single vehicle crash on Hwy 20 near mile post 134. Preliminary investigation reveals that a GMC Acadia, operated by Moriah Shackelford (28) of LaPine, was westbound when the vehicle left the roadway and struck a raised driveway causing the vehicle to become airborne. There were four juveniles in the vehicle: 2 were treated locally for injuries; 1 was transported to Portland by air ambulance; 1 sustained fatal injuries and was pronounced deceased at the scene. Shackelford sustained minor injuries.
