On December 22, 2016 at approximately 6:20 AM Oregon State Police Troopers responded to two vehicle fatal crash on Hwy 214 MP 19. The preliminary investigation revealed a westbound 1994 Toyota pick-up was being operated by Pedro Topete (age 35 from Jefferson). Topete vehicle struck black ice and crossed the centerline colliding with a 2006 Peterbilt semi-truck. The driver of the semi was identified as John Illingworth (age 47 from Keizer). Topete was pronounced deceased at the scene. Illingworth was not injured. The highway was closed for approximately 6 hours. The semi-truck was not loaded at the time of the crash.
