On September 8, 2015 at about 5:20pm, OSP Troopers and emergency personnel responded to the report of a truck vers us a pickup on Highway 99W near milepost 105 (just north of Junction City). Emergency personnel arrived on scene to find a 2007 Ford F150 severely damaged in the roadway and a 2006 Kenworth with trailer rolled over on the shoulder. Emergency crews had to extricate the driver of the Kenworth, Randall G LIKENS, age 56, of Chesire, from the truck. He was taken to McKenzie Willamette Hospital for minor injuries. The operator of the Ford, Cory A WALL, age 32, of Eugene, was taken by ambulance to Sacred Heart Riverbend Hospital. There is no information on his injuries at this time. A 3 month old male passenger was also taken to Sacred Heart Riverbend Hospital. There is no information on his injuries at this time. A passenger in the Ford, Amanda D WALL, age 28, of Eugene, was pronounced deceased on scene. A 2 year old male passenger was taken by air ambulance to a local hospital where he was declared deceased. Preliminary information indicates the Kenworth was towing a box trailer and was traveling southbound on Highway 99W when the truck drifted off the highway and onto the shoulder. The truck and trailer reentered the highway where the trailer slid across the centerline into the northbound lanes (into the path of the northbound Ford). After colliding with the Ford, the Kenworth and box trailer exited the highway and rolled onto its roof. This is an ongoing investigation.
